Dr. Thomas "Tom" D. Holleback, DDS left us unexpectedly on Friday, December 13, 2013 at the age of 61 years.

Tom was born on April 17, 1952 in Milwaukee to Darrell and Joan (nee Zimmerman) Holleback. Tom graduated from Marquette Dental School in 1976 and began his dental practice in Hubertus. He married Ann Kartes on November 2, 1996 at Holy Angels Catholic Church in West Bend. He went on to be very involved in the community in the years to come.

Tom loved the outdoors and was an avid hunter and fisherman. He was a huge fan of football, especially the Green Bay Packers and Wisconsin Badgers. Tom also loved his Marquette Warriors!

Tom is survived by his loving wife, Ann. He was a devoted dad to his four children Katie Holleback, Becky Rands, Alison (Adam) Ruback, and Dan Schmidt. He is also proud grampa to Paige Ruback. He was a loving son to Darrell and Joan and dear brother to Patti (John) Brandenburg. Tom will be dearly missed by his father-in-law Harold Kartes. He is further survived by many nieces, nephews, in-laws, other relatives, and dear friends. Tom is preceded in death by his mother-in-law Jane Kartes and brother-in-law Tom Kartes.

A Memorial Service will be at 7:00 PM on Tuesday, December 17, 2013 at the Phillip Funeral Home Chapel, 1420 W. Paradise Dr. in West Bend with Fr. Jerry Repenshek presiding. Gathering will be at the funeral home from 4:00 PM until 6:45 PM on Tuesday. In Tom's honor, please feel free to dress casual.

In lieu of flowers, gifts to the family are appreciated for future purchase of a memorial.
