Jerry Michael Kozlow, age 78, passed away on August 18, 2024 at The Kathy Hospice in West Bend. He was born on March 4, 1946 in Chicago, IL to Jerome and Ruth (nee Moffitt) Kozlow.
Jerry enjoyed photography, cars (as of late, electric cars) and especially his family.
Jerry is survived by his wife of 54 years, Kerry; children, Jonathan and Daniel; sister-in-law, Vicki (Scott); cousin Pam (Wayne) He is further survived by other relatives and friends.
Jerry is preceded in death by his parents, Ruth and Jerome.
To honor Jerry's wishes, no formal service will be held.
